Docket - immunization web site.

My two shot are not shown on the site search.

---says to contact local health department.   Livingston or Maplewood?


I got the card so probably not a problem but if Docket becomes a verification source that is more accepted than the now-counterfited card....


My shots weren't found either. When you click the More Info button it directs you request a change to NJIIS Immunization records using this form which you can download to a PC and fill in and save.

You also need a file with your drivers license or other state or government issued ID.

Then it tells you to go here and submit a ticket.

It prompts "What can we help you with" and since none of the answers supplied really apply you can just pick tahe first one. Fill in the form and upload the two files (the change request form and your ID).

You should get an email telling you to do the search again in Docket. This time it found my records. However it required a verification code showing the last two digits of a phone number. In my case it was no phone number I'd ever had, so I replied to the email saying that and giving them my actual cell phone number. They replied that the change that record, and now I was able to complete the process.

I couldn’t find my info until I dropped the hyphen in my last name ( which is my legal last name).  Then it loaded.  It is hyphenated on my vaccine card.


Does anyone know if the Docket app is being accepted for proof of vaccination by Broadway theaters?


Check the spelling of your name on your card, and also your DOB.  They may have made a mistake.  My eldest got his vaccine recently after turning 12 and it showed up on Docket after less than a week.
